Official Transcripts & Certified Diploma Copies & Duplicates

Enrollment & Degree Verifications - Verification of degrees awarded or attendance is completed through the National Student Clearinghouse (NSC). All third parties who wish to have a verification completed for any student must go through NSC.

Registration - The Registrar’s office facilitates the scheduling of courses for students and provides academic plans. Each trimester all University students are required to complete a term registration process to confirm they wish to return to CUKC for the upcoming trimester. This process is initiated through CUKC email. If you have questions regarding your academic plan, please reach out.

Course Add/Drops - Course Add/Drops: Before deciding to withdraw from a course, students are highly encouraged to explore Academic Support options such as time management, test taking strategies and tutoring as well as meet with their instructor to determine what future homework, quiz or test grades are required to pass the course. The Registrar’s office is available to discuss how adding or withdrawing courses may effect a student’s academic plan. If a student has determined a course drop is in their best interest, they may complete an Add/Withdraw form and visit the Registrar’s office to process their request. - Note: A student may withdraw from a particular course one time

Students must repeat the course the following trimester and must remain in the course for a grade
Course withdraws may compromise future financial aid eligibility
Course withdraws may delay graduation

• Verification Letters: Outside entities may request specific academic information on CUKC letterhead prior to your graduation date. If you need a Jury exemption letter, or have an insurance provider, a lender or other 3^rd party that requires confirmation that you currently attend CUKC or your potential graduation date on official letterhead, please complete the Verification Request Form (please make a link) and email it to registrar@cleveland.edu. • Name Changes - Should you have a legal name change during your attendance at CUKC, it is critical that you update the University and complete the name change process via your student portal as outlined below. CUKC requires legal names to be represented in our Student Information System and on diplomas to ensure we provide accurate academic documents. As an academic institution CUCK is required to verify diplomas for state and other national agencies for the purposes of employment, licensure and insurance. Therefore, CUKC must be able to accurately identify graduates and match student information to academic documents.

Name Change Process:
Log into your student portal
Go to the Academics Menu on the left hand side
Click on Forms & Resources
Then select name change request
You will need to upload a copy of the legal documentation for the reason for your name change and a new social security card showing your new legal name

Address Changes - Please be sure to update CUKC with your current address by completing the address change process via your student portal as outlined below. You may also add emergency contacts via this process.

Log into your student portal
Go to My Profile
Go to My Information
Click on the Addresses Menu tab

To enter a new address, click on the ADD A NEW ADDRESS button

Select the address type
Current – use this to update your address
Emergency Contact – use this to alert the University who to contact
Enter your new address information
Save

FERPA - Family Educational Rights and Privacy Act Information

This Federal law protects the privacy of student educational records. The University may not discuss any aspect of a student’s education without specific consent. In order to grant the University permission to speak to anyone regarding a student’s education they must identify the individual via the FERPA process within their student portal as outlined below.

Follow directions as outlined in your portal

Notary Public – Notary services are available in the Registrar’s office free of charge

National Board Exam Registration and Approval: Students set up an account through the National Board of Chiropractic Examiners (NBCE) online portal and request to complete an exam. NBCE then contacts the Registrar’s office for approval and once approved, NBCE will alert the student they may select and pay for their exam date. Registrar exam approvals are completed once per day. Click on the link below to go to the MyNBCE Online Account Portal login. You may reach out to the Registrar if you have questions about NBCE board eligibility and the specific coursework required at registrar@cleveland.edu.

Eligibility for Part I, II and Physiotherapy are determined by CUKC and require completion of specific coursework.
In general students are eligible to take part I when they complete Tri 5.
In general students are eligible to take part II when they complete Tri 8.
Physiotherapy may be taken after students complete PHT648 and PHT688.

Eligibility for Part III and IV are determined by NBCE.
Students must be within 9 months of graduation to take Part III and students must have progressed to Tri 8.
Students must be within 6 months of graduation to take Part IV.

Go to Your Class Schedule
The Class Schedule Calendar gives you a M-F calendar view of when you will be in class. Use the PDF Schedule Report at the top of the page to see detailed information such as room and instructor information.

- Final Grades:

Log into your student portal
Go to Academics
Go to View My Grades
Select the program and Term to view grades
Note: Grades are available on the last day of the 8-week term for College of Health Science students and the last day of the Trimester for DC students.
